Squeeze, Hold, Release Your Way to Sexual Health

This article first appeared on the Good Therapy website.

Have you heard of pelvic floor exercises? You may have learnt about them from your aerobics instructor. Or perhaps your urologist was the one who ordered you to squeeze your butt cheeks together? Maybe your gynae was the one who asked you to attempt to tighten your vagina, or was it your anus?

I have news for you: They’re all the same thing. This exercise is more commonly known as the Kegel exercise.

Named after Dr. Arnold Kegel, the Kegel exercise consists of contracting and relaxing the muscles that form part of the pelvic floor. The muscles actually being squeezed are known as the pubococcygeus muscles (or PC muscles for short) at the pelvic floor.

There are many benefits to doing Kegels. For men and women, it helps strengthens pelvic floor muscles weakened due to aging, from being overweight, or for those who have a chronic cough, or a genetic predisposition to weak connective tissue.

Women who experience pelvic organ prolapse or urine leakage due to pregnancy and childbirth benefit from doing Kegels. Also women who have persistent problems reaching orgasm find Kegels sensitise their pelvic muscles. For men, Kegels aids in better ejaculatory control and can help treat prostate pain and swelling.

To locate your PC muscles, imagine how it feels when you need to pee, but for whatever reason (say you are in a meeting), you are unable to immediately do so. Or try to stop the flow of urine while you are actually using the bathroom. That’s right! Those are the muscles. After contracting those muscles, use your mind to relax them, you should feel your pelvic floor move back down to the starting position.

I can simplify the ‘How’ into three keywords:

  • Frequency – If you have never done any Kegels before, chances are your muscles are not well developed. Use a “squeeze, hold, release” pattern. Do so for a minimum of three sets (10 times per set) daily. If you are doing them correctly, you should feel some slight ‘burn’ or tiredness around your outer thigh.

  • Duration – Within days, you should be able to build up from duration of one second up to three seconds. If you can get to a count of 10, even 15, seconds, repeating the “squeeze, hold, and release” – that would be excellent!

  • Intensity – As your muscles become stronger — and you become more experienced with the exercises — this movement will be more pronounced. This is when you have the ability to not only contract on the surface level, but squeeze even further. Much like how bodybuilders who lift weights push themselves just that bit more, you can also do that for your pelvic region!

Kegel exercises can be done anytime, anywhere. They are easy to do and require no special equipment. There is no reason not to do them, considering the benefits to bladder and bowel control and also sexual function! Give it a try!

Testimonial: Another Vaginismus Client

"I always believe in saving myself for the person I marry and I have never really been bothered by sex. Until I met the one and what I thought was a natural process of just doing it turn out to be such a traumatic and painful session. I could not, cannot have penetrative sex! I was demoralised as a woman and my fiance was full of anger as he felt that I did not love him enough to do it. We nearly called off our wedding. Having a session with Martha was our last resort, last straw in saving our relationship.

Through 4 sessions with Martha, who is very compassionate and sensitive, she gave us a very professional and realistic pet talk, explaining and managing our expectations, anger and disappointment. She gave us a lot of space to air our grievances and issues that we were suppressed inside us. With her as a bridge, a knowledgeable moderator, we work out our inner differences and expectations. My relationship with my fiance took on a deeper level and our sexual expectations with each other were clarified.

Our last session with Martha was a practical session where Martha taught us relaxation techniques and different exercises to strengthen and relax ourselves for sex. We were also educated that sex does not only means penetrative sex, there are many different ways and techniques of pleasures.

I did the exercises and using the breathing and positive self-talk mental techniques Martha taught me, I finally managed to have penetrative sex. Although it was not as romantic as whatever the sexy stuff shown on TV, it was a feat for me to be able to JUST DO IT instead of tensing with fear!

Martha who is a very warm and generous therapist, has constantly encourage and kept in touch with me, telling me that sex takes practice and it will become better and better with each try and she is right! Sex is becoming more and more enjoyable for me and I am now able to speak truthfully and plainly to my husband what works for me and what do not work for me. With Martha's coaching, my husband has also improved in his techniques for making me feel comfortable and making it more enjoyable.

I thanks Martha for making it a possibility for me to get married! And also provide us with understanding how male and female sexual expectations are different and how to manage each other expectations to make a relationship work. I am thankful that there is a sexologist in Singapore! A female one to boot, that allows me to talk candidly without any reservations and who totally understand that feelings and emotions are important aspect of sex too! Keep up the great work, Martha!"

Laser Circumcision

This article first appeared on Good Vibrations Magazine.
http://www.blogger.com/img/blank.gif
You know you are a sexologist when a little advertisement tucked away in the sports section of a commuter newspaper in Singapore catches your eye.

It reads Laser Circumcision for Male Adults and Children and what is even more interesting is that one can use Medisave for it. Medisave Scheme is a national healthcare savings scheme designed to help members pay hospitalisation expenses incurred in Class B2/C wards in restructured hospitals in Singapore. Medisave savings can also be used to pay for certain outpatient treatments like chemotherapy, radiotherapy and dialysis.

In short, circumcision is a minor surgery which is actually approved by the local government! One could argue that since circumcision is not an actual medical need, the use of Medisave funds should not be allowed.

Google male circumcision singapore and you are in for more surprises! Apparently Singapore is known as a "location of circumcision clamp design and manufacture". Two circumcision clamp designs that operate on the “Clip-&-Wear” (ischemic necrosis) principle are associated with Singapore. The original version of the SmartKlamp was produced here, at the time when the development was being conducted by the Dutch company Circumvent BV. Entirely separately, the Pro Ring was designed in Singapore, but it appears never to have gone into commercial production. A very similar design, the Ismail Klamp subsequently emerged in the Malaysian town of Johor Baharu, just across the causeway from Singapore. Fascinating stuff!

How about some actual statistics? According to this article Global circumcision rates by Jake H Waskett published on CIRCUS.org or Circumcision Independent Reference and Commentary Service, 15 percent of the men in Singapore (14.9% are Muslims, and 0.1% are Jewish) are circumcised. That is 345,284 out of 2,309,590. That is a lot less than the 79 percent circumcised men in the United States, but an interesting fact to me nevertheless. Since this article was last updated July 14, 2011, it is probably the closest I can ever get to learning about circumcision rates in Singapore.

Most people only think of male circumcision when they hear of the word circumcision. Many are not aware that female circumcision exists. You can get a good picture of the state of female circumcision from this page hosted on the website of Association of Women for Action and Research (Aware) in Singapore - the only gender equality advocacy non-profit in Singapore.

In case you do not know, a more acceptable term for female circumcision should be female genital mutilation (FGM). According to World Health Organisation (WHO), female genital mutilation is the designated term for any procedures under which female genitalia are wholly or partly removed or damaged, whether for cultural reasons, or for any reasons other then medical. FGM was introduced because female circumcision was considered by many victims to trivialize the issue. (Diries, 2005: 212) Female genital cutting, female sexual mutilation and ritual genital surgery are other words used to describe the same procedure or group or procedures.
The term FGM provides an emotional force behind a policy of zero tolerance towards any cutting that may occur in the female genital area… The label also carries the assumptions that all female circumcision practices intentionally limit female sexual pleasure in order to control women, have a detrimental effect on women’s health, and because they harm women and girls, must be eradicated regardless of the extend of the procedure or the context in which they occur. Moreover, the policy of zero tolerance framed by the UN and advocated for adoption by member states suggests that the educated first world (from which this term emerged) has the responsibility to ‘save’ third world women – without recourse to their very different histories and ‘manifestations of differently structured desires’.” (Aby Lughod, 2002: 783; Walley, 1997; Wood, 2001 in Newland, 2006: 119)

WHO estimates that the global impact of these practices constitute a serious public health crisis. As many as 2 million girls a year are subjected to some form of cutting, adding to a cumulative worldwide count of 130 million women and girls. Egypt, Ethiopia, Kenya, Nigeria, Somalia, and Sudan account for about three quarters of all cases. In some of these countries, more than 90 percent of the female population has been cut. (Gollaher, 2000: 189)

Outside Africa, FGM is practiced in Oman, both North and South Yemen, the United Arab Emirates, Bahrain, Qatar, and some areas of Saudi Arabia. Some indigenous people in Peru, Colombia, Mexico and Brazil are reported to practice clitoridectomy. Muslim populations in Indonesia and Malaysia and Bohra Muslims in India, Pakistan and East Africa also practice FGM. (Dorkenoo, 1994: 23). Let's not forget that it exists in Singapore too! In Singapore, the FGM procedure involves using stainless steel scissors to make "a small nick" of a centimetre or so on the skin (or Type I).

At this point, you may be wondering where I am taking this. FGM is strongly linked to virginity, chastity and fidelity, which are often prerequisites for marriage, and usually surrounded by various ceremonial celebrations and coming-of-age rituals. In societies where marriage for a woman is her only means of survival, it would be difficult to persuade her to relinquish the practice for herself or her children. Zero tolerance policies might only lead to FGM being carried out covertly, to perhaps much more detrimental effect to the women involved. Campaigns for its eradication will have to be prosecuted through education and the provision of an alternative focus for celebration in the various communities.

But we are referring to modern day Singapore where it has been argued that there is no religious basis for circumcision, and also no laws regulating the practice in Singapore. Is it really a matter of traditions die hard? If this is a matter of personal choice, it is not quite the same as plastic surgery for beautification, is it? How do we justify the use of Medisave for non-medical use then? Are we not discussing about the need for circumcision as a society because it involves our private parts, and therefore should be kept private? This above advertisement has led me to wonder all this and more.

"Before i met Martha, sex always a nightmare for me. After married for one year plus, having sex become a most stress issue in my marriage. Always feel pain, scare and tired for keep trying, but always end with crying and upset.

Until I read an article about that lot of woman having same problem like what I had facing, and that's the first time I knew Martha from the magazine, I decided to give a try to seek for her help. Through out the first session, she try her best to understand my problems and my worries, then encourage me make me feel that there is a hope that I can overcome this problem, as long as I follow her instruction, doing kegel exercises with the understanding how to make myself relax, slowly then I should be able to overcome it.

After first session, Martha always follow up my status by email and encourage me if have any problem not sure can just give her a call. After second session, finally me and my husband manage to have sex for the first time! I still feel painful and not really comfortable but at least it was no longer a barrier for me. As long as I keep practicing those exercises, it will become better and I would be able to enjoy more for my sex life.

Really appreciate Martha for helping me to become a real woman, and enjoy good quality sex life without pain and fear."- Anonymous Lady

Liberate Photobook

Internationally renowned Singapore photographer Marcus Mok has published Liberate, his long awaited first book of sensuous male photographs. The many fans of his work can now add his book to their collection of his fine art prints on their walls. Mok’s work has been acquired by the Singapore Art Museum and the Kinsey Institute in the USA, where they are considered important chronicles of Asian photography

Mok, 45, says, “I chose Singapore to launch my book because it is my homeland, it is where my inspiration and many of my models are from, and, not least of all, so my aging mother, to whom I have dedicated this gorgeous opus, can attend.” He adds that further launches are planned for Europe, the USA and Australia later this year.

Liberate is a collection of Marcus’ favourite works from the last decade of his photography. Some have been exhibited previously and hang in private collections around the world. Others are being seen for the very first time in this book and will be available as archival quality fine art prints.

In the words of Catherine Johnson-Roehr of the Kinsey Institute, USA, “Marcus Mok is a highly creative artist whose photographs display his talent for composition and his ability to use lighting and location to showcase both the beauty and the power of the male figure.”

Justice Michael Kirby of the Australian High Court called Marcus a “gifted photographer”, and you can understand why when you see the 120 pages of sensuously yet discreetly posed and photographed buffed young men, mostly Asian. There is a lightness and elegance to his touch that beguiles you into soaking up the images, seeing more and more in each one the longer you look. This is not heavy handed, in-your-face, one-look-and-you’re-done, photography – it captures the discretion of the Asian form while subtly exposing its hidden sexuality for those who want to see.

William Yang, the doyen of photography, says in the foreword to the book that “the work has a powerful aesthetic, reinforced by the book’s design. Its strength lies in its discretion.” Liberate is designed by award winning designer Jeff Tan around the theme of liberation, with quotations and poems about liberty being interspersed throughout. The result is thought provoking and challenging. Indeed, it’s inspiring to think of those who have championed liberty in all its aspects down through the generations, and to see the struggle for liberty – or its full unbounded expression – in these beautiful images.

The photographs are gorgeously printed in Singapore by Nic Ong at Indibooks on high quality Forest Stewardship Council graded 200 gsm Lumisilk acid free paper on 20 micron FM screening.

Marcus says he will leave it to his readers to divine their own personal quest for liberty and hopes they can find inspiration and strength from his work. Liberate is more than just a coffee table book and visual archive of Marcus’ journey; it is a symbol of the universal desire to be free.

About Marcus Mok

From teaching himself the art of photography, Marcus’ work has been acquired by the Singapore Art Museum, The Kinsey Institute in USA, and other valuable private collections around the world. His work has been exhibited in Singapore, New Zealand, Sydney and Melbourne and has been published in international magazines and books in USA, Germany, Hong Kong, London, Japan, Singapore, Sydney and Melbourne

After obtaining a Bachelor of Science in Estate Management from the National University of Singapore and working for 5 years in real estate, Marcus was able to realise his dream of embarking full-time on his love of photography. He has specialised since in photographing people, and after being a successful wedding photographer, moved to stock imagery. His work appears in publications and advertisements around the world through international stock agencies.

Marcus endeavours to reveal the grace and beauty of the human form without resorting to saccharine images on the one hand, or overly explicit poses on the other. He has striven to show that beauty can be revealed – and even enhanced – using creativity and subtle techniques.
Liberate can be purchased from his website here.

Local buyers only need to pay $100 including postage. You will get your copy within a week.

Elephants Have Genitals Too

This article first appeared on Good Vibrations Magazine.

I came across this article titled “Walking among gentle giants” in My Paper, a free bilingual commuter publication on Thursday, Aug 11, 2011. The same story can be found here.

I have just returned from Cambodia where I saw female elephants used to transport tourists around, so I took a double look at the photo journalism piece. I knew they were females because standing by the roadside, watching these elephants walk by, all you needed was look up to see the big folds of their vulva dangling and gently swaying side by side as these gentle giants walked on.

I was struck by the caption of this particular photograph though: “NO DETAIL TOO SMALL: Alert zookeepers find a wound among the folds in Komali’s reproductive area. Medication is applied to manage the condition. Without the strong bonds between keepers and animals, such close inspection of the elephants would not be possible.”

Whoa! Wait a minute. Are those genital warts? Of course, animals have sexually transmitted infections too. I was so fascinated that I just had to share it here.

How about the carefully worded “reproductive area”? Since genital or genitalia refers to “the organs of the reproductive system”, wouldn’t it have been more appropriate? Or would it be too offensive for a mainstream publication to use a more accurate term for fear that some member of the public would lodge a complaint about the offense? Our genitals are used for more than reproductive, but pleasure as well. When will the media get it? What does it say about our state of sexual education or sexual openness?

Nope, no mention of that or who Komali has been having sex with, or how the genitals of the latter look.

Sex Madness

This is a typical sex exploitation film from the early 1930s--complete with wild parties, sex out of wedlock, lesbianism, etc. A chorus girl's exposure to the "casting couch" also exposes her to syphilis.

Exploitation filmmakers hoped to capitalize on the taboo subjects of venereal disease, sex before marriage, lesbianism, etc. while skirting the Motion Picture Production Code of 1930 which forbade a film from containing such content. Films like this would tour the United States for years - mostly being shown in rundown, skid row theaters. This film has been re-edited and re-titled ("Human Wreckage", "They Must Be Told", "Trial Marriage", "About Trial Marriage") many times to attract the same audience to film, to take advantage of a taboo subject which may have gotten press recently or to appease local censors who disapproved of the film's content.

Year: 1933
Director: William Curran
